Phys. Ed Waiver

Physical Education Waiver Requirements were adopted by the Perkins Board of Education in 2010.  This allows a student to waive the required .50 credit in Physical Education by participating in an approved activity.

Approved activities include a completed season of the following:

  • Cheerleading
  • Marching Band
  • Athletics

The student will not receive a grade or credit for this course.  Students must successfully complete two separate seasons, and submit two separate waiver forms in the approved activities in order to waive their physical education requirement.

A waiver form for each season must be completed and signed by the student, parent, and coach, advisor or director.  The completed Waiver Form must be submitted to the guidance office after the end of the season, but no later than the Friday immediately following the Sports Banquet for that season.

It is the responsibility of the student to turn in this waiver form to the guidance office.
